Backpacks For Kids Supply Request 2025 - 2026

The Northshore Schools Foundation is here to help you!  
If your family is experiencing financial hardship, the Northshore Schools Foundation would like to provide your student(s) with a free backpack and school supply kit for the 2025–2026 school year. 
 
This form is for parents/guardians to complete for their Northshore student(s). Please complete the form by [date].  
 
IMPORTANT INFORMATION 
  • Supplies are basic and may not match classroom-specific supply lists. 
  • This form is for 2025-2026 Northshore School District students only. Enrollment will be verified by staff member 
  • Once we verify enrollment, you’ll receive a confirmation via email and text. 
  • One form per household, up to 4 students. 
  • If you are requesting supplies for more than 4 students, please complete a second form. 
  • Backpacks cannot be requested by phone or email. This form must be completed to receive supplies. 
 
BACKPACK OPTIONS 
Backpack and supply kits come in three categories - Preschool through kindergarten, 1st grade through 5th grade, and middle/high school.  
 
BACKPACK PICK UP INSTRUCTIONS 
This year, families will NOT be picking up at Pop Keeny Stadium. 
  
Backpacks will be available at the school/program your student will attend in the 2025–2026 school year. 
Location, date and time of pick up:  To Be Determined, all registered families will be notified as soon as location is confirmed.
We will send you a text and email reminder the week prior to pick up.
